Community participation in Cranbourne and nearby areas

Spend time in the places and activities that are meaningful to you. Community participation is about being part of places and activities you choose. That may be a regular group, a shopping trip, a local event or simply more confidence getting out. We begin with your interests, not a fixed activity list.

What it can include

Support with the details that matter

The right arrangement depends on your goals, the tasks involved and how you want assistance delivered. These are examples to discuss, not a fixed package.

Support to attend appointments and programs

Plan the practical help needed to take part once you arrive.

Building confidence in community settings

Build familiarity and confidence at a pace that feels right.

How we work with you

A plan that fits real life

Where support happens

A worker can assist with preparation, travel, communication and practical support during an outing. We can also help explore accessibility, opening times and other details that make the plan workable.

Before we begin

Before an activity we agree the destination, assistance required, schedule and any transport costs. Afterwards, your feedback helps shape the next outing.

Funding and fit

Clear arrangements before the first visit

NDIS funding can be self-managed, plan-managed or NDIA-managed. Tell us how your relevant supports are managed and we will confirm the proposed service and payment arrangement before work begins.

We also agree the tasks, worker match, schedule, travel arrangements and relevant costs. Where clinical care is involved, assessment and staff capability are checked before the service is confirmed.
