Make a referral to KeyPoint Care

Helping someone find the right support starts with a clear, respectful conversation. We welcome enquiries from participants, families, support coordinators and health professionals.

We will discuss the requested services, location, funding and any clinical requirements before confirming whether we can offer support.

A short description of the tasks, goals and preferred days or times helps us understand the referral.

Location and funding

Tell us the participant's suburb and whether the relevant NDIS funding is self-managed, plan-managed or NDIA-managed.

Clinical considerations

If nursing or complex care is requested, tell us what assessments or care instructions are already available.

Only share personal or health information through an approved, secure referral process. Please do not put sensitive details in an initial email.

After an enquiry

What our team will do

We review the request, speak with the participant or authorised contact, clarify support and worker requirements, and discuss whether the service can be provided in the requested area. If it is suitable, we explain the proposed schedule, costs and next steps before anyone agrees to begin.

A referral does not create an automatic service commitment. We will be clear about availability and any additional information needed for safe planning.
